Access the 1095-C Form in OnTrack

OnTrack is a secure site that allows employees to view their 1095-C form(s) electronically.

If an employee gives consent to receive the electronic 1095-C form then no paper form will be issued.

Navigate to OnTrack to access your 1095-C form.

1095-C Electronic Consent

Click the 1095-C tab and click Change Preferences

  • If an employee gives consent to receive the electronic 1095-C form then no paper form will be issued.
  • If an employee withholds consent then the ability is lost to view/print any year, including the current year.
    • If there is a need to view/print the official (with SSN) form for the current OnTrack session only, then give consent, view/print the document, return to the consent form, and revoke consent.

Select your choice and click Save

1095-C Form

Click on the year link to view the 1095-C form and a new window or tab will open.

If needed, employees are able to print the form using the browser settings.

The current year will only be displayed once the Payroll office releases the 1095-C's in mid to late January.

If consent has not been given to view the 1095-C electronically then a message will be displayed.

Click Change Preferences to go to the 1095-C Electronic Consent form to give consent.

Errors Viewing PDFs

Windows

Firefox:

  • Does not work with default settings
  • Workaround - set Adobe Acrobat Pro or Adobe Acrobat Reader as the default application to open PDFs

Chrome:

  • Opens using the default Chrome PDF viewer

Internet Explorer:

  • Opens using the browser's default viewer
  • Using Internet Explorer could have issues in regards to fonts. If fonts are not displaying correctly, please try another browser.

Mac OS X

  • This saved PDF file does not open in Mac OS's default PDF viewer (Preview)

Safari:

  • By default, Safari uses Preview as its default PDF viewer (which does not work)
  • Workaround - Set Adobe Reader as the default application to open PDFs - Enable Adobe Plug-in in Safari

Firefox:

  • Does not work with default PDF viewer. You receive a notice: "This PDF document contains forms. The filling of form fields is not supported. Option to "Open With Different Viewer"
  • You can select to open the file Adobe Acrobat Pro or Adobe Reader if installed

Chrome:

  • Works with default PDF viewer.

iOS

Safari:

  • Does not open with default PDF viewer
    Tap on the top of the page below the address bar to activate an "Open In" option and select supported app (Adobe Acrobat or Fox-it Reader)

Firefox:

  • Does not open with default PDF viewer
    No workaround found

Chrome:

  • Does not open with default PDF viewer
  • Click 3 vertical dots to activate menu
  • Click "Request Desktop Site"
  • Then tap on the W-2 which activated an option on the bottom to "OPEN IN"
  • Select supported app (Adobe Acrobat or Fox-it Reader)

Workarounds Requiring Install

  • Install Adobe Acrobat Reader app and tell browser to open file in pdf viewer app
    or
  • Install Fox-It Reader and tell browser to open file in pdf viewer app
